She is survived by her husband, Michael Phelps; two children, Chad Phelps and Tommy Phelps; father, Norman Hosmer; and many other family members and friends.
In lieu of flowers, the family asks that memorial contributions may be made to the funeral home to help pay for funeral expenses.
